Submit medical Records

Submit Your Medical Records & 

Complete our secure online intake form covering your medical conditions, work history, daily limitations, and treating providers. Our HIPAA compliant form. Once submitted, your case is assigned to a dedicated Case Coordinator who will be your single point of contact throughout.

Introduction Call

Your Case Coordinator Introduces Themselves Within 48 hours of your intake, your assigned Case Coordinator calls & messages you to introduce themselves, confirm they have your information, explain the timeline, and answer any immediate questions. You will know exactly who your person is from day one and they stay with you through delivery.

Physician Evaluation

We schedule your appointment with a contracted licensed physician in your area. Before you arrive, we prepare a medical review packet for the physician to review. The physician examines you, reviews the documentation, and signs the RFC form. This is the most critical document in your SSDI file and it is included in your fee.

Packet Delivery

After our 3 layer quality review your complete documentation packet is delivered securely to you. Your Case Coordinator walks you through exactly what is in it, how to submit it, and what to expect from the SSA timeline. You submit it to the SSA directly North Path does not file on your behalf as we are not a representative service.
